Recent claims in the media that the “big 4” tech firms (Google, Amazon, Apple and Microsoft) will ‘disrupt healthcare’ are often met with justified expert skepticism. Over the past couple of years, it is with common consensus that these companies do not fully understand the idiosyncrasies of the healthcare industry at an operational level. The provision of healthcare to patients is not a business model that generally fits in with the model of how big tech firms operate and scale. Operating healthcare businesses is generally very local and high touch, requiring relatively high fixed costs and capital investment for lower returns. On various occasions, they have publicly announced their ambitious and ‘disruptive’ healthcare endeavours but with very few making it past the proof of concept.
While big tech will unlikely go into care delivery as a primary priority, they are strongly positioned with their business and technological strengths to play a major supporting role for healthcare provider organisations. The catalyst for this is due to an increasing focus on patient-centred, data driven value based care. This is due to increase in healthcare spending (in the trillions in the US). Big tech see these increasingly healthcare costs as a massive business opportunity. Technology enabled healthcare providers will have a potential competitive advantage and even be a threat to those who provide care under traditional fee for service models. Despite big tech’s capabilities to significantly improve user experiences of clinical information systems and help reduce clinician burnout, it is unlikely that big tech will be able to disrupt this space. The majority of healthcare organisations have heavily invested financial and human resources in current practice management software, and the switching costs to another vendor are astonishingly high. In addition, big tech companies are increasingly not trusted by the public with health data, as a result of recent consumer data privacy and security breaches and government-led antitrust investigations. According to a 2018 US survey by Rock Health, only 11 percent of people were willing to share their health data with a tech company like Amazon or Facebook. Interestingly, the public would rather share their health data with the federal government, insurers and big pharma. Not surprisingly, they are most trusting to share their health data with their doctor. So what are the “big 4” tech firms currently doing in the healthcare space? Google’s primary interests in healthcare appear to be population health, structured data and artificial intelligence. The company is applying artificial intelligence to disease detection, data interoperability and potentially health insurance. Projects include using image processing to screen for diabetic retinopathy, early sign screening for blindness, and early detection of breast cancer via their subsidiary company DeepMind Technologies. In 2014, they acquired DeepMind for a reported $500 million (USD). They have also recently invested $375 million (USD) into a technology focused small health insurer in the US called Oscar Health. They aim to revolutionise health insurance utilising big data and telemedicine. Google AI quotes on their website that they “think AI is poised to transform medicine, delivering new, assistive technologies that will empower doctors to better serve their patients.” Recently, Google DeepMind announced that its artificial intelligence technology can detect acute kidney disease 48 hours before doctors can spot it. This condition is apparently difficult to spot by doctors early as the symptoms and signs are often not clear, leading to potentially fatal kidney injuries including dialysis. Amazon Amazon’s particular strengths are likely their Amazon Web Services (AWS) cloud infrastructure, Alexa AI voice assistant technology and their deep supply chain and delivery capabilities. Amazon have always been interested in healthcare, with its first notable attempt of investing into drugstore.com in 1999. This was an e-commerce play in the pharmacy space which was acquired by Walgreens in 2011 for $409 million (USD) and eventually wrapped up as Walgreens decided to focus on other priorities. Recently, Amazon acquired an online pharmacy called PillPack for $753 million (USD) and later announced a partnership with JPMorgan Chase and Berkshire Hathaway, to develop an independent healthcare company to look after their US employees and associated family’s health, with the goal of improving employee satisfaction and reducing costs. Apple Apple’s strength is in consumer facing products. The big tech company successfully secured one of the 9 exclusive spots in Food and Drug Administration’s (FDA) Digital Health Pre-Certification Program. This program run by the US government evaluated applicants for their robustness and quality control protocols, to certify them for the development of future medical technologies. Apple is also involved in many university research collaborations but notably, the Apple Health Study with Stanford University. This study utilises the Apple Watch’s heart rate sensor to collect data on irregular rhythms and alert users who may be experiencing the heart condition atrial fibrillation. In 2018, Apple signalled the entrance into the electronic health record space by launching HealthKit API. An Application Programming Interface (API) simply allows independent software applications to talk to each other with certain defined constraints. This enables users to share their clinical and personal health data with developers via trusted smartphone third party apps. Apple have also entered the healthcare payment space with their solution Apple Pay with recently announcing their partnership with US health insurance company UnitedHealthcare. Microsoft Microsoft have focussed their resources on their trusted healthcare IT infrastructure through solutions such as Azure (like Amazon Web Services) and big data analytics capabilities to advance precision medicine. Precision medicine is "an emerging approach for disease treatment and prevention that takes into account individual variability in genes, environment, and lifestyle for each person." (Precision Medicine Initiative). In 2017, Microsoft setup a healthcare division at a Cambridge University research facility in the United Kingdom. The work at this research lab focuses on the application of big data, artificial intelligence, machine learning and cloud computing on healthcare problems. Microsoft’s healthcare division boasts a multi-billion dollar resource with claims to have over 1,100 employees, 168,000 customers and 14,000 partnerships. Final thoughts The “big 4” tech firms are already playing a critical role in influencing the healthcare industry globally, and this will be even more prominent over time with the pressing focus on value based healthcare. Products and solutions from such companies will increasingly become more distributed and sophisticated as technology and the quality and volume of data improves. Nonetheless, they also have some key immediate challenges, which is firstly to win over consumer trust with their solutions, where data privacy and security concerns are of greatest importance to clinicians and their patients. It will be interesting to see over the next decade in regards to how healthcare will be transformed by technology, and how the big 4 tech firms will collaborate with the industry while supporting startups and new innovations. Optimistically, we will see as a byproduct, a significant improvement in quality of life of all stakeholders including clinicians, patients, researchers and administrators. The culture of digital health encourages us to understand our health, get involved in activities that may prevent illness in the future, get to know our body and then act if a problem arises. This culture applies to the health of women. We all should be pursuing regular exercise and a healthy diet. In addition we should be focussing on mental health and performing activities that benefit it. Specific to women’s health are also how your periods are affecting you, pregnancy issues and breast health. Technology is providing us with solutions to all these aspects of health and more. So How Can Technology Help? So let’s focus on issues specific to women. There are many questions that we come across as health professionals about the health of women. Periods, sexual health, contraception etc. The topic of conversation often varies according to the age of the person. Younger females come in to discuss contraception and period advice, whilst older females often like to discuss things like hormone replacement therapy (HRT). A useful site to consider when asking questions about women’s health is Clue. They have an encyclopaedia of information on most topics. Free and easy to access, it is a useful starting point for anyone who needs information to learn about their body. Period Tracker In addition to this they have a period tracker. It is useful for women to track their periods from a general health point of view but also if they are trying to get pregnant. It is a simple to use tracker that can be downloaded to your phone and accessed at all times. It also tracks your mood to determine if there is a hormonal cause to any mood issue. I have a patient who was started on strong antidepressants 5 years ago by another doctor. It was not really working that well and we went through the cycle of increasing her dose, getting her seen by a psychologist and encouraging her to exercise. One day she decided to track her mood in relation to her periods and realised that her mood was typically worse prior to the onset of her period. We got her off the antidepressant and onto the pill and she was transformed. Her mood was much improved. Unfortunately it had taken 5 years to get to that point. It highlights the point that if you understand your body well, we can treat any problems accurately. Breasts This brings us to our next point which get to know your body, physically. What do I mean by this? We all have little lumps and bumps that are normal for us. It is important to know where these lumps and bumps are and monitor for change. In relation to women’s health this applies to your breasts. Get to know them! It is really important to understand what is normal for you. If you know this, then you will be identify a change. If you notice a change, you can act by seeing your doctor, who can identify if there is a problem or not. With all problems if it can be diagnosed early, the chance of cure is high. Daisy Wheel is an app that can teach you how to examine your breasts but also remind you to set aside a few moments on a regular basis to perform a check. A simple piece of tech that can really make a difference to your health. Pelvic Floor The pelvic floor is a series of muscles that sit in the pelvis and help with all activities that the pelvis performs such as controlling your bowels and bladder and sexual function. Because we cannot see it we often forget it is there. As we age, they become weaker. If you have children, they are often affected in childbirth. Strengthening your pelvic floor can provide benefit whatever stage of life you are in. Strong pelvic floor muscles can improve your sensation during sex and the strength of your orgasms. Chronic obstructive pulmonary disease is a condition where there is long term damage to the lungs causing the pipes in the lungs to become narrow. This makes it hard for air to travel in and out. The commonest cause is smoking where toxic smoke damages the pipes. Symptoms that people experience are a cough, shortness of breath and the production of mucus on a daily basis. The effect of this is difficulty in doing your day to day tasks such as working, going to the shops etc. This condition makes you prone to picking up nasty chest infections which needs to be treated quickly and effectively to prevent serious illness. How Can Tech Help? For the motivated patient, once the condition has been diagnosed, it is important to identify with your doctor, what the cause could be. Once done, make a change. Stop Smoking Often the cause is smoking: I am fully aware that stopping smoking is hard but I guarantee that if you don’t make a change now, your lungs will get worse. You need to find something inside you that will drive you to make a change. It does not matter what it is. My father-in-law is an ex smoker. He stopped overnight. How? His son who was 6 at the time said to him, “I don’t want you to die daddy.” That was enough for him to stop. Technology can support you and the My Quit Buddy app can be used to help but the key thing is that YOU want to make a change. Find that motivating factor and maximise it to kick the habit. Understand What You Are Dealing With The myCOPD app is a very useful resource for information as well as many other things. We have suggested it because it enables you to understand what condition you are dealing with. If you understand, you feel in control. If you feel in control you avoid the feeling of helplessness that many people with a long term problem suffer from, which often then affects other aspects of their life such as their mood. Empowering yourself with knowledge will make you aware of how to slow progression, how to identify that a problem is developing before it becomes serious, how to treat the condition and so on. I have said this on many occasions in my blogs: read it, learn it, own it! Identifying A Problem Your first aid kit needs to be updated. A few pieces of simple technology can open up access to healthcare. Many of these items of technology can be used for other health issues but they certainly have a role in COPD. With this condition infections are common. If not identified or dealt with quickly it can end up with a hospital admission. Having the tools to gain objective information about your health can make a real difference. Two simple items - a thermometer and an oxygen monitor can provide sufficient information that a problem may be brewing. Someone with a high temperature and low oxygen levels and COPD needs to see a doctor soon. It could indicate that you have an infection that may be affecting your lungs ability to absorb oxygen. Add in a digital stethoscope and you can get a GP online to listen to your chest and decide what intervention is needed. Do you need to leave work now and see your GP face to face? Is it the cancer or something else? I used to work in a famous cancer hospital. I, then and now, always say to people, if you have any questions, write them down. Doctors are busy and when you go to see them, you have a small window to discuss issues that may or may not be a problem. The number of times I have been told by patients that they have had numerous questions but their mind has gone blank. It is a wasted opportunity and take it from me both patient and doctor do not want to miss a problem because it was not reported. This is why we have highlighted the CancerAid app. It puts everything in one place so you can track your symptoms and treatment as well as find information that may be useful. A diagnosis of cancer is undoubtedly going to affect your mood. That is the reality of the situation. If the condition is progressive, you want to ensure that the time you have left is enjoyable. You can see someone where and when you need to in the comfort of your own surroundings. Often taken for granted until too late, heart health should be an important focus in our daily lives. It is a daily issue in my life and a key driver to what I eat, how I exercise and how I live due to the fact that my father passed away aged 63 due to a heart attack. He was fit and well. Didn’t drink or smoke but didn’t really exercise. To look at, you would think he was healthy (certainly by 1990’s standards), but little did we know what was going on inside. Heart health has various aspects and it is important to break them down to get an understanding of what you can focus on. Prevention Diet Anyone who has a passing interest in health will know about the role of diet in health. Today there is plenty of focus on fats (good vs bad) and well as the role of carbohydrates, (large amounts of bad carbohydrates in your diet will increase your risk of diabetes which in turn increases your risk of heart problems). There are plenty of food tracker apps and health coaches that can guide you to maintain a healthy diet and looking through our website will identify a few that should be considered. Exercise “Cardio” is the type of exercise we need to focus on when we are talking about heart health. What do we mean by this? It is any exercise that gets your heart rate up and makes you sweaty and slightly short of breath for 30 mins, 5 times a week. It is a lot of exercise and take it from me, life often makes it hard to meet this target. What is important is that you find something you enjoy that you can easily incorporate into your life for the next 30, 40 or 50 years of your life, (I really mean this!). We are talking about sustainable lifestyle changes. It can be anything, running, swimming, ballroom dancing, it doesn’t matter. As long as you enjoy it and can do it often, then that will work. Many of my patients have busy lives, kids and long commutes and often state because of this, they can’t exercise. It is however, important to think out of the box. For example, if you have a long commute on the train, then take a pair of running shoes to work. On the journey back, jump off the train a few stops early and do a fast pace walk or run home. Do this Monday to Friday and then forget about it, your exercise is done. Technology in the form of apps and wearables can track your levels of activity and motivate you to keep doing more, (see our health and wellness section for more information). Health Checks When I was young, I remember my father going to see his friend who was in the hospital as he had just had a heart attack. I remember asking Dad, how old his friend was, to which my Dad said “36 years old”. That seemed really old to me at the time but now I realise it was very young. Today, heart disease in 36 year olds is rare, so what has changed? Health checks have been introduced where your cholesterol and blood pressure are checked and lifestyle changes are advised to prevent a problem in the future. Sufferers will tell you of the daily struggles of dealing with the pain, the need for regular strong pain relief and the frustration of not being able to do any task easily. It affects your mood, your relationships and your ability to work creating a vicious cycle. I see patients despairing. They get momentary relief from medication but then the pain always comes back. They always ask if there have been any new discoveries or breakthroughs in the field of chronic pain that may help them. I cannot say that at present there is anything revolutionary coming out soon, but the digital health industry is working on it. It is important to understand that we are complex organism and if one part of us is not working properly, it will affect other parts of us. Chronic pain can be knee osteoarthritis, cancer related or from previous injuries. To treat it, we have to look at the individual as a whole. Their physical problems, their social problems but most importantly their mental wellbeing. How can technology help? The first way to control any issue is to understand it fully. If you understand it you can own it! You can then target your treatment accordingly. A great resource for understanding pain is the painHealth website. Read it. Learn about pain. Understand what you can do about it and be in control of the situation. If you feel you are in control, you do not feel helpless or overwhelmed. They often state whilst they won’t stop the pain entirely, they “turn down the volume”. We are seeing an opioid crisis globally and I believe technology will be the source of alternative strategies to challenge the use of strong medications. Whilst we may not be quite there yet, progress is certainly being made. Looking at the effect pain has on your mental health is vital in treating long term pain problems. Depression and anxiety are common. Knowing that the pain is relentless can wear you down. Everyday you wake up you are in pain. Simple things like going to the shops seem like major obstacles. The one thing I would say is that feeling low or anxious is normal in this situation, so don’t beat yourself up about it. However this is where technology has made significant progress, so there is hope. Seeing a psychologist has now become easy and accessible online. Low back pain is a very common health condition worldwide and a major cause of disability in all age groups. It can impact the level of functioning at work and life in general. The economic impact of lower back pain in the United State of America has been estimated to be over 100 billion dollars annually and growing. Lower back pain can be classified as acute (less than 6 weeks), sub-acute (6 to 12 weeks) or chronic (> 12 weeks). Causes of low back pain In most cases, the cause of low back pain is unknown or difficult to pinpoint but multiple risk factors have been identified contributing to the development of lower back pain which include:
Popular medical management of low back pain Radiological imaging such as x-rays, CT scans and MRIs of the lumbar spine are often unreliable and unhelpful in determining the cause of your lower back pain. Popular medical treatments include pain killers (analgesics) and anti-inflammatories despite a lack of scientific evidence associated with its efficacy carrying potential side effects, as the causes of lower back pain are rarely addressed. Preferred evidence based treatment options include physical therapy (physiotherapy), joint mobilisation and manipulation, exercise rehabilitation, cognitive behavioural therapy and pain re-education. Surgical intervention is generally the least preferred option, and considered when conservative non-invasive management fails. When to seek medical attention? There are also wearable devices which help improve your posture and core muscles at work and at home, notably, UprightGo. There are many exercise rehabilitation apps which assist in providing exercise programs to prevent and rehabilitate lower back pain conditions for example, Kaia’s back pain app, which in a recent study, demonstrated that it was more effective than traditional therapy and online education. Using wearables or activity trackers such as Fitbit, Garmin, Apple’s step counter, Strava or RunKeeper apps can help with behavioural change and promote good exercise habits. All of these products have broader implications for physiotherapists when managing their patient’s general health and optimising clinical outcomes. Health professionals such as physiotherapists are traditionally guided by established science and evidence based practice guidelines. Therefore, the self-quantification movement may seem in conflict with this due to the experimental nature of this approach and relatively smaller population sizes applied. Nonetheless, there is an increasing number of health professionals such as physiotherapists who are looking more closely at how to extract meaningful insights from this data from self-tracking to optimise health outcomes at both an individual and population level. In particular, many of the measures from such solutions can assist physiotherapists in behavioural change and promoting healthier lifestyles. What are the implications for physiotherapists managing patients who are self-quantifiers? From one perspective, self-quantifiers are ideal patients as they are engaged and proactive in taking responsibility over their own health. However, it is important to be aware that being unprepared for such scenarios can result in patients challenging the physiotherapist’s knowledge, and as a result, threaten their authority. So in such circumstances, it may be best for the physiotherapist to take a strong collaborative and work with the patient as partners in their health, focusing on helping their patient achieving their goals, ensuring that the patient is informed of the potential benefits and risks associated with what they are experimenting with, and ensure avoiding any potential danger and safety issues. An example of self-quantification often used in the community are activity tracking solutions such as the Strava app, Fitbits, Apple and Garmin watches. This has been reported to be a useful tool for physiotherapists as it can provide insights into external training load and assist with injury prevention and performance optimisation. In addition, the online social community associated with many of these apps which often incorporate benchmarking performance can be helpful for motivation and support. A back pain app outperforming physiotherapy? With an increasing number of freely available articles on the internet such as “The back pain app out-performing physiotherapy” backed by a randomised control trial, physiotherapists should start to take serious note of the longer term implications of how emerging digital health solutions may impact their practices and patients. Without going into the details of the limitations of this study, it recently demonstrated that patients using the Kaia app reported significantly less pain than the control group which comprised of weekly physiotherapy and online education. Both groups were encouraged to incorporate an active lifestyle and perform a home exercise program. The Kaia app comprises of 3 therapy modules: back pain education, hands off physiotherapy, and mindfulness/relaxation techniques. Users of the app also have the ability to interact with a human coach via the app if they have any particular questions. This study adds to the emerging body of evidence which supports a “multi-modal approach” to chronic pain management. Traditionally, multi-modal treatment approaches to chronic pain management is often costly, limited to a small number of specialised medical facilities and appropriate clinical team. The results of this study provide promise that emerging digital health technologies have the potential to provide viable alternatives to traditional models of care, providing better accessibility, affordability and in many cases, more effective treatment solutions for specific populations. Comparing the latest results from consumer surveys from previous years, there appears to be a shifting trend from interest in low risk entry digital tools that assist with fitness and wellness towards those that assist with managing health conditions - “with the intention to address concrete health needs”. This trend suggests that healthcare providers will increasingly need to be aware of the emerging consumer use and inquiry of these tools, as the data collected from such devices can potentially assist health professionals in providing patients with further insights into their condition. Another interesting trend is urban consumer adoption of telemedicine services is more than twice as likely than rural consumers. Not surprisingly, tech savvy millennials are the target market of many of these telehealth startups, who are seeking convenience in engaging in healthcare services like the way they engage in food delivery and transportation services. Rate of Perceived Exertion Guides for Aerobic and Resistance Training It is important to understand that to optimise performance, there needs to be a fine balance between training hard and overloading resulting in injuries and comprising performance. This sounds like common sense however to accurately monitor training loads is a challenging task given that it is associated with frequency, duration and intensity. Duration and frequency are relatively easy to measure however intensity could be measured by for example, how fast you are running, your heart rate, how much weight you are lifting with the barbell or the complexity or speed of the movements you are performing. Tracking your training loads is not just about managing the specific exercise you are doing but can help you determine when it may be appropriate for you to do some cross-training or vary your load. Acute on Chronic Workload Ratio (ACWR) For those who are a serious about injury prevention and performance optimisation,a great way to find your training ‘sweet spot’ while minimising the risk of injury is by using the Acute on Chronic Workload Ratio calculator. Scientific research indicates a 0.8-1.3 ratio range is ideal. Injury risk increases at a 1.5 ratio or higher. To calculate your ratio, multiply your rate of perceived exertion (RPE) after each exercise with the duration, intensity or frequency of the exercise session (external load). So why the big fuss on hypertension (high blood pressure) Hypertension is one of the most important cardiovascular disease risk factors due to its high prevalence and significant medical costs. Approximately, 78 million Americans (33 %) have hypertension [systolic blood pressure (SBP)≥140 mmHg and/or diastolic blood pressure (DBP) ≥ 90 mmHg] (Mozaffarian et al 2015). Similarly, according to the Australian Heart foundation, in Australia these figures are similar with close to 6 million or 33.7% of the population suffering with hypertension. Individuals 50 years of age or younger with pre-hypertension have double the lifetime risk of stroke, heart failure, coronary heart disease, and intermittent claudication compared to individuals of the same age with normal BP. (Mozaffarian et al 2015). Fortunately, exercise has been widely recognised as a method to reduce hypertension. As mentioned already, lifting heavy weights temporarily increases blood pressure. Paradoxically, high levels of chronic physical activity and aerobic exercise training can prevent some of the age-associated rise in blood pressure seen in many normal humans, and exercise training can also lower blood pressure in those with hypertension. I want to lift weights – how do I do it safely? First and foremost, you shouldn’t lift heavy weights or even engage in a vigorous exercise program unless you are cleared by your doctor and had a chance to discuss any precautions or special considerations. As a general guideline, it is recommended that you shouldn't lift weights if your blood pressure is uncontrolled and higher than 180/110 mm Hg. If your blood pressure is lower at 160/100 mm Hg you shouldn’t lift heavy weights without consulting your doctor (Sorace et al 2014). When someone gets a diagnosis of diabetes, it is often thought of as a condition where your body cannot control it’s sugar levels. This is correct, but patients are often left confused as to why we as doctors start worrying about a variety of issues such as your blood pressure, cholesterol, your feet or your eyes. Often I hear, “But my cholesterol is not very high, so why are you starting me on cholesterol medication, doc?” or “Why are you saying I need to worry about my blood pressure when there is a problem with my blood sugar?" Background A better way to explain diabetes is that yes, your body cannot control your sugar levels very well, but it is the effect of the high blood sugar on your pipes that transport blood around your body, that is the problem. These pipes are in your eyes, feet, heart, kidneys….pretty much everywhere. The high blood sugar can damage those pipes resulting in those parts of the body not working very well. If you don’t control your blood sugar, then your eyes, feet and kidneys become damaged. We worry about your blood pressure and cholesterol because if these are high, they can also damage those pipes causing more damage and more chance that those parts of your body won’t work well. So, how can we keep things under control? The most important thing to learn when given a diagnosis of diabetes is what to eat and what to avoid. By changing your diet you can reduce the chances of the condition worsening. Numerous apps are available that can help you log what you are eating so you can better understand what is good for you or not. In addition, many apps come with a health coach who can advise you on any questions that may arise. Should I eat a sweet potato if I have diabetes? Is low fat yoghurt better for diabetics? Advice on diet, changes regularly and it can be hard to keep up to date, however having access to a health coach can make your life easier. Weight Weight gain is often an indicator that what you are eating is not correct, but this is not always the case. Monitoring your weight is very important in diabetes and very easy to do with a simple bit of technology. However with my patients who are motivated and start exercising regularly when given the diagnosis of diabetes, they often come in despairing because, despite them eating well and exercising they are putting on weight. Why is this happening? With a healthy diet and exercise, you often put on muscle and lose fat, but muscle is denser than fat and causes you to put on weight. Having a set of scales that can tell you your weight and body fat percentage can be useful. If you are aware that you are putting on weight but losing fat, then you know your lifestyle changes are working. If you are putting on weight and gaining fat, then more changes need to be made. Blood Pressure We mentioned above that controlling all things that can damage the pipes that transport blood, is key to preventing the diabetes from progressing. Blood pressure, often known as the silent killer, rarely has any symptoms until it is too late. So checking it often will allow you to do something if it starts to creep up, (change your diet or take some medication). Usually you would see your doctor every six to twelve months, but in the digital age we as doctors should encourage ownership of the problem by the patient. Monitor your blood pressure at home and seek help when needed rather than wait around until you see your doctor every once in a while. By acting sooner you will prevent any problems from causing significant damage. Blood Sugar Many people with diabetes require insulin (the hormone that reduces your blood sugar level). This must be injected into various parts of the body, however the dose of insulin may depend on the level of your blood sugar at the time. This is worked out by piercing your finger and measuring the level of sugar in your blood. For many diabetics, needles and injections have become a part of their life. The digital health revolution has however brought hope to many sufferers. Continuous glucose monitors (CGM) are devices that place a fine needle under the skin that continuously measures the level of glucose in the fluid that bathes the cells under the skin (interstitial fluid). They provide a continuous reading of your blood sugar levels and remove the need for regular finger prick testing. These can be coupled with another device that continuously pumps insulin under the skin according to the readings from the CGM. What you have here is an artificial pancreas (the organ that normally produces insulin in the body). Such devices have significantly improved the quality of life of many diabetics, however they are not freely available and must be commenced under the supervision of your specialist. Emergencies

Many parents know, as children get older, one problem gets better, but then another comes along. Learning how to deal with each issue makes life so much easier. Emergencies Next, as a parent we all secretly worry about emergencies. What do you do if your child starts choking or burns themselves? It is imperative that all parents learn what to do and the British Red Cross app provides good advice. Download it and learn. There is no excuse. As a parent you are responsible for your child and knowing how to deal with health emergencies is essential until the ambulance arrives. (Remember, if in doubt, call an ambulance). Thermometers Emergencies aside, I guarantee you that one day your child will be unwell: a cough, an ear infection or diarrhoea. Being exposed to these bugs helps your child to develop their immune system, but as a parent you need to support your child back to good health. One of the best pieces of kit that I bought was the thermometer. I was at my mums with my children one day. My mum says to me, “I think your son has a fever.“ I put my hand on his forehead like parents do, “No mum, he’s ok”. She ignores me and gets a thermometer to check his temperature. She gives me that look that mums do, “ It’s 38.4” (that’s high!). It made me realise, get as much objective information as possible.
